Dr. Ir. PANGARIBUAN, Adrianus., MT., CFEI
Dr.Adrianus Pangaribuan holds a Doctor in engineering, majoring in Mechanical Engineering with sub study in Fire & Explosion Engineering from University of Indonesia. He hold a degree in Electrical Engineer also from University of Indonesia majoring in Electrical Engineering. Throughout his career he has gained significant experience. The first 2 years he spend as maintenance engineer, and then 20 years experience as site engineer, site manager, project engineer, senior engineer and technical director in the Mechanical & Electrical field. He works for the Building, food Industries, mining industries, chemical industries and oil & gas industries, offshore and onshore facilities. He has been working with national and international clients. He has a good communication skill, cooperative, good time management and problem solving oriented. Since 2007 based on his field experience and formal education he is beginning to work in fire & explosion investigation field but limited to mechanical explosion such as BLEVE, Dust explosion, Electrostatic, and Deflagration and get the certificate as a Certified Fire & Explosion Investigation (CFEI) from NAFI (National Association Fire Investigation) – USA and the first Indonesian who get this certificate. As a Fire & Explosion Investigator, he knows and understands how to protect and prevent the fire and explosion from the design by conduct the Process Hazard Analysis ( HAZOP, HAZID, SIL, LOPA, etc) until implementation under his supervision.
Yuni Kusminanti, BSc (Public Health), MSc (Psychology)
Yuni is graduated from Public Health majoring in Occupational Health & Safety, she is then taking the Master for Industrial & Organizational Psychology at Universitas Indonesia. Her experience started as the associate researcher at Centre of Assessment & Application of OHS at Faculty of Public Health, Universitas Indonesia since 1998 until now. During 2000-2007 she was a lecturer at Department of OHS, Faculty of Public Health, Universitas Indonesia. She is also a researcher at Disaster Research & Response Centre (DRRC) Universitas Indonesia since 2014. Now, she is the Occupational Safety & Health Management Coordinator at Universitas Indonesia (UI) at which she is responsible for managing OHS through out UI campus, including UI OHSMS, Contractor Safety, Health & Environmental Management System (CSHEMS), Safety Transportation, SHE Promotion, and Canteen Hygiene. She is a professionals in HSE Management System, Behaviour Based Safety, Safety Culture Development & Assessment. She has been working with many industries, government and universities in the field of HSE, Enterprise Risk Management & safety culture
IR. Soehatman Ramli
Soehatman Ramli has more than 35 years experience in the Occupational Health and Safety for Oil and Gas Company. He was EHS Manager at PERTAMINA IV Cilacap as well as in Marketing & Trading Pertamina. He graduated for Diploma of Safety Management from British Safety Council and loss control Institute from Atlanta, USA. He also has educational background of Chemical Engineering, Graduate of Safety Management from Universitas Indonesia and Magister of Business Administration from IMNI Jogjakarta. He was Chief of Indonesian Safety Association (IAKKI), Head of OHS Competency Standard Development (SKKNI) and Head of Competency Certification Agency (LSK K3). He is now the Head of Independent National Safety Team in Oil & Gas. He is also an active Lecturer at Universitas Indonesia, STIKES Binawan & Universitas Sahid. He is also a representative in World Safety Organization for Indonesia.
Dr. Ir. Waluyo Martowiyoto QCRO, QRGP.
He is currently a Member of Civil Services Commissionairs (Komisi Aparatur Sipil Negara). He has an experience in oil and gas industry with lots of network in Government Institution and State Own Company of Indonesia. He is an expert in Safety Management System and Business Ethics. He has strong leadership and great motivator. He is also the author The Risk Behaviour Safety Handbook. He graduated on 2010 from
Universitas Indonesia (UI) for Doctoral degree in Strategic Management. He holds a Magister Management, Business and Mechanical Engineering from
Prasetya Mulya Business School and Trisakti University. He has been a Member of Civil Services Commissionairs (Komisi Aparatur Sipil Negara since 2014 until now. He is also an Owner Rege Tour and Travel 2012-2014, he is. Lecturer in Binus Business School during 2012-2014. Since Mar 2010 – Jan 2013 he was a Chairman at PT Pertamina Trans Kontinental. During Mar 2008 – april 2012 be was a Director of General & Human Resources (Direktur UMUM & SDM PT PERTAMINA (Persero)). During Sept 2009 – Dec 2009 he was a Chairman in Indonesian Corruption Commision (KPK)
Adonis Muzanni, BSc (Chem), Master of Engineering Management (UTS, Sydney)
Adonis is the Managing Director of PT Muzanni Lestari since 2013. He graduated from Chemistry at Universitas Indonesia, and Master in Engineering Management from University of Technology Sydney, Australia. He has more than 20 years of experience in managing production process & manufacturing, including shoes production, plant development and chemical safety. He has been working with many industries. He is now a professional trainer & consultant in the area of production quality improvement, business process improvement, chemical safety & security, chemical inventory management and business continuity management. Muhammad Dawaman, MOHS
Mr. Dawam graduated from Universitas Indonesia, Master of Occupational Health & Safety. He is a member of HSE Indonesia & Indonesian Independent National Safety Team (Oil & Gas). Several projects he have been working including earthquake research, safety culture, business continuity, Ergonomic & Psychology hazard at Indonesia Power, ergonomic hazard at PT Meares Soputan Mining Minahasa Nort of Sulawesi Under WLN, Fire & Explosion, and Safety Culture at PLN. He has more than 15 years experience in industries, both as trainer and consultant. He has experience as Training Coordinator of Environment, Safety, and Health in the industry. He is having background of educator (online) also in safety management. He is familiar with Environment, Safety, and Health International Standard. Since 2016 – present, he is a HSE Specialist PT Parker Hannifin Indonesia & PT Pusaka Tandi. During 2015 – 2017 he is the Coordinator Assistant in Emergency Response at Universitas Indonesia. In 1998 – 2014, he is thr Operation Supervisor PT Energizer Indonesia. During 1994 – 1998, Warehouse Supervisor PT Cahaya Sakti Furintraco and during 1993 – 1994, Project Leader Milk Consumption Survey of Bandung Society.
Puguh Mahendrodjati
Mr. Puguh Mahendrodjati has experience from a background in process engineer and process safety engineer for wellhead platform design; Oil & Gas Industries (Onshore & Offshore) Engineering design, well-versed in all phases of business process (i.e. Conceptual design, Front End Engineering and Detailed Engineering) and being involved in all areas of project management from conceptual design through construction to commissioning and hand-over to operation.
He has experience as Process Safety Engineer are follows as developing management of action plan and provide recommendation for final closure of all defect findings and re-instate a safe operating envelope on offshore facilities, help evaluate risks and cost-effectively resolve PSI issues, including recommendations from PHAs, audits and incident investigations, facilitator of HAZID/HAZOP/ SIL workshop, perform PHA studies (i.e. HAZID/ HAZOP/ SIL studies) and safety studies/ dossier (i.e. FERA, EERA, ESSA, QRA) for some project, conduct firewater demand and hydraulic analysis, day to day to support subsea and operation team, interact with operations and maintenance and interface with external and internal regulatory bodies. Evaluate whether management systems of PSM and its element are performing as intended. It complements other PSM control and monitoring activities in elements such as leadership and engagement, performance measurement, and inspection work activities that are part of the asset integrity elements, establish system of formal periodic assessments at section, corporate, and external levels, review data from audits and assessments to assess system-wide weaknesses and gaps, provide tracking of corrective actions and disseminate results from audits to enhance organizational learning.
He has previously experience as Process Engineer including developing process simulation model using Aspen HYSYS software, process equipment / instrument sizing and associate data sheet preparation, relief system contingency (Pressure safety valves) analysis and system depressurizing analysis accordance to API standard, performing flare header sizing and selection calculations using FLARENET, performing thermal radiation and flare stack sizing using FLARESIM and develop/ review drawing documents (i.e. PFD, P&IDs, SAFE Charts and ESD logic).
He has skilled in numerous computer software programs including Aspen HYSYS, Flarenet, Flaresim, PHAST, PHAST Risk, Pipesim, Pipephase, Pipenet, some instrumentation software (i.e. InstruCalc, Masterflow and CCI software) and Microsoft Office Application (i.e. Ms. Excel, Ms. Word and Power Point).
