However, these “reviews” and “audits” may not be sufficient to provide an organization with the assurance that its performance not only meets, but will continue to meet, its legal and policy requirements. To be effective, they need to be conducted within a structured management system that is integrated within the organization.
In Indonesia, company which have more than 100 workers or have a high potential risks for incident is compulsory to implement Indonesian Occupational Health & Safety Management System (OHSMS). It is based on Indonesian Government Regulation PP No 50 Year 2012 about OHSMS. Since its declaration on 2012, all companies which classified under this regulation should comply with this. The Process Safety Management (PSM) is process hazard analysis – a careful review of what could go wrong and what safeguards must be implemented to prevent any accident happen. Covered employers must identify those processes that pose the greatest risks and begin evaluating those first. PSM clarifies the responsibilities of employers and contractors involved in work that affects or takes place near covered processes to ensure that the safety of both plant and contractor employees is considered. The standard also mandates written operating procedures; employee training; pre-startup safety reviews; evaluation of mechanical integrity of critical equipment; and written procedures for managing change.

An incident is an unplanned or undesired event that adversely affects a company’s work operations. Incidents include work-related injuries, occupational illnesses, property damage, spills, fires or near miss events that could have resulted in any of these. All incidents should be investigated. An incident that results in a serious employee injury, considerable property damage, a major fire, or spill obviously warrants an extensive investigation. A minor incident or near incident also requires a thorough investigation and may reveal significant potential for a serious incident if the conditions are left uncorrected. A thorough investigation will reveal the root cause of the incident. The purpose of an incident investigation is to determine the work element and root causes of incident, and to assist in providing the company with a solution to prevent recurrence.

The SVA is a systematic process that evaluates the likelihood that a threat against a facility will be successful. It considers the potential severity of consequences to the facility itself, to the surrounding community and on the energy supply chain. The SVA process is a team-based approach that combines the multiple skills and knowledge of the various participants to provide a complete security analysis of the facility and its operations. The objective of conducting a SVA is to identify security hazards, threats, and vulnerabilities facing a facility, and to evaluate the countermeasures to provide for the protection of the public, workers, national interests, the environment, and the company. With this information security risks can be assessed and strategies can be formed to reduce vulnerabilities as required. SVA is a tool to assist management in making decisions on the need for countermeasures to address the threats and vulnerabilities.
